How to Create an Account to Access the Patient Portal<\/h2>\n

If you don\u2019t have a Patient Fusion patient portal account, you\u2019ll need to create one to communicate with your provider and access your medical records online.<\/p>\n

Please read this carefully before creating your account.<\/strong> \u00a0Patient Fusion verifies your identity by matching the information you provide when creating your patient portal account with the information that our office has on file. \u00a0You will need the following information that MUST EXACTLY MATCH<\/strong> the information our office has on file: first name, last name, date of birth, email address, and mobile phone number.<\/p>\n

To create your Patient Fusion patient portal account, follow the steps outlined below:<\/h3>\n

1.<\/strong>\u00a0We will send an invite to the patient portal via the email address we have on file. Please note this email MUST BE A VALID EMAIL ADDRESS<\/strong>. Please check junk or spam\u00a0 folders. The email typically takes about 5 minutes to appear in your inbox. Open this email, and click \u201cAccess Your Records.\u201d<\/a><\/p>\n

2.<\/strong> A new web browser window will open up. Verify your identify by selecting either \u201cEnter PIN received from the doctor\u201d or \u201cRequest security code via phone\u201d.\u00a0If you choose the PIN, you’ll need to contact the doctor’s office and have them give you the PIN. If you select, \u201cRequest security code via phone\u201d, the phone number you enter on the next screen\u00a0MUST EXACTLY MATCH THE MOBILE PHONE NUMBER<\/strong> we have on file.\u00a0<\/a><\/p>\n

3.<\/strong> If verifying identify using \u201cRequest security code via phone\u201d, enter all of the following fields: \u201cPatient\u2019s date of birth (MM\/DD\/YYYY)\u201d and \u201cYour phone number from the doctor\u2019s record\u201d (this is the mobile phone number that we have on file). Select the method you would like to use to receive this security code: text or voice.\u00a0<\/a><\/p>\n

4.<\/strong> A temporary, 5-digit security code will be sent to the mobile phone number on file. Enter this code, and select \u201cCheck code\u201d.\u00a0<\/a><\/p>\n

5.<\/strong>
\nSelect \u201cCreate a new account\u201d or \u201cLink to an existing Patient Fusion account\u201d. If linking to an existing Patient Fusion account, you will need to know the email address and password of the patient account that you would like to link to. If you do not know this information, please contact our office at (425) 947-1970 for assistance.\u00a0<\/a><\/p>\n

6.<\/strong> To create a new account, you will need to complete all fields shown below and will need to create a password for this account. PLEASE RECORD YOUR PASSWORD AND ALL OTHER ENTRIES IN A SAFE PLACE FOR FUTURE USE. Passwords must have at least 8 characters, 1 uppercase letter, and 1 number or symbol. When all fields have been entered, click \u201cCreate new account\u201d.\u00a0<\/a><\/p>\n

Your Patient Fusion patient portal account should now be created, and you\u2019ll be able to communicate with your provider and access your medical records through this patient portal.
